Allied Health Services is the division of APRN® that provides comprehensive home health services. AHS was formed in 1982 when APRN® identified the need for skilled, comprehensive home health care services. Since then, our goal has been to develop a growing team of first rate home health care managers and professionals. The AHS staff provides the best in caring, competent and evidence-based outcomes to rehabilitate our patients and return them to an independent, productive life style.

The changing trends in health care delivery are showing patients being released from hospitals sooner than ever before. The reason....third party payers are relying on more cost efficient methods to achieve the same high quality of patient outcomes. Allied Home Health Services offers patients and third party payers a chance to partner with each other to achieve these common goals.

Both you and your patients will appreciate the ease of dealing with only one organization. Allied Home Health professionals stay abreast of the latest rehabilitation and skilled nursing procedures, and truly care about the people we assist. We are unique in that we offer all the comprehensive services within one agency. In doing so, we are able to assure continuity of care and ongoing collaboration with the attending physician in order to more effectively coordinate the individualized services needed by each patient.

In addition, we accept Medicare/Medicaid, worker's compensation and almost all major third party payer sources. Our experienced staff can readily determine eligibility for coverage and options available by Allied Home Health Services.

Award Winner

Allied placed in the top 10% in the state of Ohio and the top 20% of home care agencies in the nation for reducing unnecessary hospitalizations for patients under their care.

Allied Home Health Services, Holland, Ohio. The agency reduced its Acute Care Hospitalization rate from a 30.2 percent at baseline to 20.8 percent. Among the best practices the agency used to improve on this measure were the following:

  • Implementing the use of a telephone monitoring tool to provide consistent screening of patient concerns with Registered Nurses responding to calls.
  • Consistent scheduling of interdisciplinary team (IDT) conferences and documentation of review on the IDT Team Conference Sheet.
  • Developing and consistently using the "Risk Factors for ACH Tool" with each start of care/resumption of care (SOC/ROC).
  • Using the "ACH Emergent Care Audit Tool."
    Front-end loading of skilled nursing visits in the first two- to three-week period of home care services.
  • Assessing immunizations at SOC.
  • Consistently educating all clinical staff.
  • Informing staff of progress and goals using positive reinforcement.
